Tyler starts a visit without checking the latest orders.
In this scene
Tyler: I'm heading to Mr. Kim's. Same visit as last week, I know the routine.
Monique: Did you read his new orders? The doctor changed his care plan on Monday.
Tyler: No. I figured nothing changed.
Monique: We only do what the current orders say. Read them before every visit, and ask if anything is unclear.
Tyler: Got it. Orders first, then the visit.

Check the current ordersBefore each visit, read the patient's current orders and care plan. Orders change, and care must follow what is ordered now, not what you remember. Ask the supervising nurse or physician if anything is unclear.
